Science Fair Forms and Publications

All ISEF publications (information, rules and forms) are available at the ISEF Document Library. You must also print the MSRSF Entry Form from this page.

This page has links to all the forms required for entering projects in the MSRSF.

All of the forms can be filled out online and printed. Alternatively, you can print the blank forms and make as many photocopies as necessary.

Forms Required for All Projects

2012 MSRSF Entry Form. One entry form is required for each project. Please fill out online and print out if possible. Otherwise, print neatly or type.

Research Plan (1A)
Approval Form (1B)

Please submit the entry fee along with the forms. The entry fees are:

Senior Division: $10/project
Junior Division: $5/project

Note: The Official Rules mention an official abstract form. This form is only required at the International Fair, not the MSRSF.

2011 ISEF Forms Required only for Grand Prizewinners

Official Abstract Form—The Official Abstract Form is required for grand prizewinners.
Of course, there are more, but we'll cross that bridge when we get there.
